Laboratory and x-ray services and radiation therapy include professional and technical laboratory and radiological services ordered by a physician and provided to a patient by or under the direction of a physician in either:(1) an office or similar facility other than a hospital outpatient department or a clinic; or(2) an approved laboratory (but only with respect to those services which the laboratory is certified to provide under Medicare).
