Subject to the specifications, conditions, limitations, and requirements established by the Texas Department of Health (department) or its designee, a physical therapist must:(1) be licensed by the Texas State Board of Physical Therapy Examiners or other appropriate state licensing authority;(2) comply with all applicable federal and state laws and regulations governing the services provided;(3) be enrolled and participating in Medicare;(4) be enrolled in and approved for participation in the Texas Medical Assistance Program (TMAP);(5) sign a written provider agreement with the department or its designee;(6) comply with the terms of the provider agreement and all requirements of the TMAP, including regulations, rules, handbooks, standards, and guidelines published by the department or its designee; and(7) bill for services covered by the TMAP in the manner and format prescribed by the department or its designee.
