SECTION/RULE §111.46 - Intern in Speech-Language Pathology License--Issuing License

(a) The department will issue an applicant, whose application has been approved, a license containing the licensee's name, license number, and expiration date.(b) An intern in speech-language pathology license issued by the department remains the property of the department.(c) The department will issue a duplicate license upon written request using a department-approved form or in a manner prescribed by the department and payment of the duplicate/replacement fee under §111.160.
