SECTION/RULE §120.70 - Responsibilities of License Holders

(a) License holders shall notify the department of changes in name or mailing address within thirty (30) days of such change(s).(b) Notification of address changes shall be made through the department's website, in writing, or by telephone. Notification of name changes shall be in writing and shall include the former and present name, license number, former and present mailing address, and a copy of the legal name change document, such as a marriage license, court order or divorce decree.
