(a) In order for the public to be informed regarding the functions of the Board and the Board's procedures by which complaints are filed with and resolved by the Board, each licensee is required to display at every location where optometric services are provided information regarding the Board's name, address, and telephone number.(b) The licensee may either display a placard or sign furnished by the Board or provide to all patients and consumers a consumer pamphlet developed by the Board containing the name of the Board, mailing address, and telephone number for the purpose of directing complaints to the Board.(c) The placard or sign shall be conspicuously and prominently displayed in a location where it may be seen by all patients.(d) The consumer pamphlet, if chosen, shall be prominently displayed and available to patients at all times.
