SECTION/RULE §281.21 - Complaints

Complaints may be filed with the agency in writing or by submitting a completed complaint form to the agency by mail or other method of delivery or through the Internet. A complaint form shall be maintained on the agency's Internet site and at the agency's office for use by a complainant. The complaint form shall request information necessary for the proper processing of the complaint by the agency, including, but not limited to:(1) complainant's name, address, and phone number;(2) name, address and phone number of subject of complaint, if known;(3) date of incident;(4) description of drug(s) involved, if any; and(5) description of incident giving rise to complaint.
