SECTION/RULE §295.1 - Change of Address and/or Name

(a) Change of address. A pharmacist shall notify the board in writing within 10 days of a change of address, giving the old and new address and license number.(b) Change of name.(1) A pharmacist shall notify the board in writing within 10 days of a change of name by sending a copy of the official document reflecting the name change (e.g., marriage certificate, divorce decree, etc.).(2) Pharmacists who change their name may retain the original license to practice pharmacy (wall certificate). However, if the pharmacist wants an amended license (wall certificate) issued which reflects the pharmacist's name change, the pharmacist must:(A) return the original license (wall certificate); and(B) pay a fee of $35.(3) An amended electronic renewal certificate reflecting the new name of the pharmacist will be issued by the board without a fee.
