SECTION/RULE §365.7 - Duplicate Pocket Card

(a) The Board shall issue a duplicate pocket card to replace any pocket card that is lost, destroyed, or mutilated.(b) The licensee or registrant requesting the duplicate pocket care shall submit to the Board:(1) an application for the duplicate stating the reasons for the request; and(2) the required fee.
