SECTION/RULE §511.93 - Applicant's Responsibility for Requesting Accommodations

An applicant seeking an accommodation is responsible for making a request for accommodation and providing documentation of the need with the Application of Intent or the Eligibility Application submitted to the board. This information shall be kept confidential to the extent provided by law. The board will evaluate each request individually, in accordance with the guidelines set forth herein, to provide an appropriate and effective accommodation. All requests to the board for accommodation shall be submitted on the prescribed form.
