SECTION/RULE §513.12 - Application for Firm License

(a) Application for a firm license must be made on a form prescribed by the board. The application must be accompanied by an affidavit of an individual owner who holds a license to practice public accountancy affirming that all statements are true and correct.(b) A firm shall notify the board not later than the 31st day after the date on which information in the affidavit is changed, including information regarding the admission or withdrawal of an owner or resident manager.
