SECTION/RULE §161.30 - Registration and Renewal

(a) In accordance with Chapter 156 of the Act, a physician license must be registered with the board and renewed every two years after it is issued.(b) A renewal notice will be sent to the physician's address of record at least 60 days prior to the expiration date of the registration.(c) The physician must:(1) complete the renewal form;(2) pay the renewal fee and any additional fees, as applicable:(A) initial biennial permit - $456.00;(B) subsequent biennial permit - $452.00;(3) verify and update their physician profile; and(4) provide any other relevant information requested.
