SECTION/RULE §177.3 - Complaint Initiation

A complaint must be made in writing and include:(1) the complainant's name and contact information;(2) the name of the licensee against whom the complaint is made;(3) the name and birth date of the patient or individual harmed, if applicable; and(4) the time/date and place of the alleged violation or action.
