SECTION/RULE §181.34 - Instructions and Requirements for Reporting Assisted Reproduction Procedures Performed by a Health Care Facility Under a Gestational Agreement

(a) Healthcare facilities in this state at which assisted reproduction procedures are performed under gestational agreements must report reproduction procedures under gestational agreements and the current status of embryos created for said procedures on the Report of Assisted Reproduction Procedures Under Gestational Agreements.(b) The Vital Statistics Unit in accordance with the guidelines of Texas Family Code, §160.763 shall prescribe the form and content of the reporting form.(c) Reporting healthcare facilities must submit this report to the Vital Statistics Unit on a yearly basis.(d) The reporting form must be submitted no later than January 31st for the previous calendar year.
