SECTION/RULE §181.63 - Abstraction of Documentary Evidence

(a) The State Registrar or his or her designated representative shall abstract on the delayed certificate of birth a description of each document submitted to support the facts. This description shall include:(1) the title or description of the document;(2) the name and address of the custodial organization, if any;(3) the creation date of the original document; and(4) all birth facts required by §181.62 of this title (relating to Documentary Evidence; Requirements and Acceptability) contained in each document accepted as evidence.(b) Original documents submitted in support of the delayed certificate of birth shall be returned to the applicant after review. Copies of all items submitted shall be maintained and indexed by the State Registrar.
