SECTION/RULE §27.17 - Provider Approval Process

(a) To become an approved provider, an applicant must submit a completed application to the department.(b) The department will review the application and provide a response within timeframes specified in policies and procedures. An application will not be reviewed and considered until all information is provided in a clear and understandable manner.(c) Providers approved by the department must also enroll with the Medicaid Claims Administrator as a Medicaid provider.(d) Providers who fail to submit an application to enroll as a state Medicaid provider within twelve months of approval by the department must submit a new application to the department.
