SECTION/RULE §97.179 - Tuberculosis Record

(a) Form and content of the certificate. The certificate is part of a multipurpose form, Tuberculosis Record, which shall be used for certification that an employee or volunteer does not have tuberculosis; a medical record of the screening, evaluation, and/or treatment of an employee or volunteer or an inmate; or as a medical transfer record for employees, volunteers, or inmates moving from one correctional facility to another. This form, or equivalent information, must accompany an inmate whenever he or she is transferred from one correctional facility to another. (b) Deadline for filing a certificate. The deadline for filing certificates is one month after the certificate is completed and signed by a physician. The certificate shall be filed with the local health authority. (c) Tuberculosis record. The Tuberculosis Record is a multipurpose report form that can be used for a Certificate, Record of Transfer, and/or a Tuberculosis History Record. The form shall be used to submit the reports pertaining to an employee, volunteer, or inmate of a jail or correctional facility.
