SUBCHAPTER C - ENROLLMENT AND RESPONSIBILITIES OF FINANCIAL MANAGEMENT SERVICES AGENCIES (FMSAS)
SECTION/RULE §264.317 - CDSA Reports

A CDSA must:(1) compile a report in accordance with the format provided by DADS addressing each service delivered through the CDS option, including the actual number of hours or units of service delivered;(2) provide the report no less than quarterly, and monthly if requested, to:(A) the employer or DR; and(B) the case manager or service coordinator; and(3) provide a copy of the report to DADS, upon request by a DADS representative.
