(a) If DADS terminates ERS, a provider may be paid for the last month of service, regardless of how many days of service were provided that month, if:(1) the provider has already conducted a system check that month before the termination of services;(2) the provider conducted a system check on the day it picked up the equipment; or(3) the provider could not complete a system check because:(A) the individual's telephone was disconnected;(B) the individual damaged the equipment;(C) the equipment was picked up at a location other than the individual's home; or(D) the individual changed his telephone number or address without allowing the provider to remove the equipment from the individual's home.(b) The provider must:(1) document the results of the final system check; or(2) document the reason the provider was unable to complete a system check.
