SECTION/RULE §282.57 - What information must the provider agency include in the request for a waiver?

The provider agency must include the following information for each staff member for whom it requests a waiver:(1) name;(2) job title;(3) specific education or experience that qualifies the staff member for the waiver; and(4) a statement that he or she is competent in providing services.
