For a designated resident who has transitioned to the community, a service coordinator must conduct post-transition monitoring activities in accordance with HHSC's IDD PASRR Handbook and using HHSC forms, including conducting a post-move monitoring visit:(1) within seven days after a designated resident transitions to the community;(2) between eight and 45 days after transition; and(3) between 46 and 90 days after transition.
