SECTION/RULE §350.239 - Destruction of Information

(a) "Destruction," as used in this section, means physical destruction or removal of personal identifiers from information so that the information is no longer personally identifiable.(b) The contractor must inform the parent when personally identifiable information collected, maintained, or used under this chapter is no longer needed to provide services to the child and family.(c) The information must be destroyed upon request of the parent; however, a permanent record of the child's name, address, phone number, service dates, delivered early childhood intervention services, and years completed and dismissed may be maintained without time limitation.
