(a) The IFSP team, which includes the service coordinator, must conduct a comprehensive needs assessment initially and annually as part of the IFSP process. The comprehensive needs assessment must include: (1) an assessment of the child; and(2) a family-directed assessment.(b) The assessment of the child must include: (1) a review of the results of the child's evaluation; (2) personal observations of the child; (3) the functional abilities and unique strengths of the child; and(4) the identification of the child's needs in each of the developmental areas listed in 34 CFR §303.21(a)(1). (c) The subrecipient must offer to conduct a family-directed assessment and comply with requirements in 34 CFR §303.321(c). The family-directed assessment must: (1) be voluntary on the part of each family member participating in the assessment; (2) be based on information obtained through the assessment as well as through an interview with those family members participating in the assessment; and(3) identify the family's resources, priorities, and concerns, as well as the supports and services necessary to enhance the family's capacity to meet the developmental needs of the child.(d) The IFSP team must assess and document the child's progress and needs of the family on an ongoing basis.
