(a) The subrecipient must assist the family with transition activities to appropriate community settings before the child exits ECI if the:(1) subrecipient determines the child is not potentially eligible for ECSE services;(2) parent chooses for the child to transition to supports and services in the community other than or in addition to the LEA; (3) parent opts out of the LEA Notification;(4) parent refuses LEA services; or (5) child is determined to be ineligible for ECSE services. (b) In compliance with 34 CFR §303.209(c)(2), the subrecipient must make a reasonable effort to convene a Community Transition Meeting that meets the requirements in 34 CFR §303.342(d) and (e) and §303.343(a), which requires the attendance of the service coordinator and at least one other ECI professional who is a member of the IFSP team who may participate through other means as permitted in 34 CFR §303.343(a)(2), and also invite: (1) representatives of the identified community settings; (2) the Blind Children's Vocational Discovery and Development Program specialist if the child has a vision impairment or the HHSC Office for Deaf and Hard of Hearing Services regional specialist if the child is deaf or hard of hearing; and (3) other program or agency representatives as appropriate.
