SECTION/RULE §356.402 - Personnel Files

A center must maintain a personnel file for each employee. Each file must include at least the following information:(1) Employment application or resume;(2) Job descriptions;(3) Signed acknowledgment of confidentiality agreement;(4) Signed acknowledgment of receipt of personnel policies and procedures handbook;(5) Performance evaluations for every year of employment in accordance with the center's personnel policies;(6) Documentation of orientation, initial training, and employee development;(7) Any status or classification change;(8) All disciplinary actions, if any; and(9) Letters of praise or criticism, if any.
