SUBCHAPTER A - OPERATING REQUIREMENTS FOR AMBULATORY SURGICAL CENTERS
SECTION/RULE §508.3 - Fees

(a) Initial license fee. The fee for an initial license (includes change of ownership or relocation) is $5,200. The license term is two years.(b) Renewal license fee. The fee for a renewal license is $5,200. The license term is two years.(c) Official submission. The department shall not consider an application as officially submitted until the applicant pays the application fee and submits the application form.(d) Nonrefundable. Fees paid to the department are not refundable.(e) Payment of fees. All fees shall be paid to the Department of State Health Services.(f) Fee schedule review. The department shall make periodic reviews of its fee schedule and make any adjustments necessary to provide funds to meet its expenses without creating an unnecessary surplus. Such adjustments shall be through section amendments.(g) Other fees. The department is authorized to collect subscription and convenience fees, in amounts determined by the TexasOnline Authority, to recover costs associated with application and renewal application processing through TexasOnline, in accordance with Government Code, §2054.111.
