SECTION/RULE §550.120 - Notification Procedures for a Change of Contact Information

Chapter Review Date A center must report a change in contact information to HHSC by submitting a change application through the online portal no later than seven days after a change in the center's:(1) telephone number; or(2) mailing address, if different from the physical location.
