Chapter Review Date (a) If a minor meets the criteria for admission into a center as described in §550.601 of this division (relating to Admission Criteria), the medical or nursing director must contact the minor's prescribing physician to schedule a pre-admission conference before the minor receives services at the center.(b) If a minor is hospitalized at the time of referral to a center, the pre-admission conference must include the minor's parent, the minor, the minor's prescribing physician, center staff, relevant hospital staff, including medical, nursing, social services, and developmental staff, and any other individuals requested by the adult minor or the minor's parent, to begin developing the plan of care.(c) If a minor is not hospitalized at the time of referral to a center, the pre-admission conference must include the minor's parent, the minor, the minor's prescribing physician, center staff, and any other individuals requested by the adult minor or the minor's parent to begin developing the plan of care.(d) A center must schedule a pre-admission conference no later than three days after the center receives a referral. The pre-admission conference must address a minor's:(1) medical history;(2) diagnosis;(3) mental and developmental status;(4) nutritional status;(5) dietary requirements;(6) functional abilities and limitations;(7) activities permitted and prohibited;(8) use of assistive devices;(9) treatment procedures;(10) use of restraints, if applicable;(11) medication;(12) safety measures to protect against injury;(13) education level and participation in an education program, if applicable;(14) immunization record;(15) receipt of services from other service providers; and(16) other appropriate information.
