Chapter Review Date (a) A center must report to HHSC the death of a minor at the center and those minors transferred from the center to a hospital who expire within 24 hours after the transfer.(b) A center must submit to the HHSC Complaint and Incident Intake Section an HHSC Provider Investigation Incident Report form no later than 10 days after the date a minor dies. A center must complete the HHSC Provider Investigation Incident Report form and include the following information:(1) name of a deceased minor;(2) social security number of a deceased minor;(3) date, time, place of death; and(4) name and address of a center.
