(a) If an agency intends to change its name (legal entity or assumed (doing business as) name), but does not undergo a change of ownership as defined in §558.23(c) of this chapter (relating to Change of Ownership), the agency must report the name change to HHSC no later than seven days after the effective date of the name change.(b) An agency must report a name change to HHSC in accordance with §558.208 of this division (relating to Reporting Changes in Application Information and Fees).(c) If an agency reports a name change, the agency must pay a fee and may be subject to a late fee, as described in §558.208 of this division (relating to Reporting Changes in Application Information and Fees).(d) After HHSC receives and verifies the required documents and information, HHSC sends the agency a Notification of Change reflecting the agency's new name. The agency must post the Notification of Change beside its license in accordance with §558.211 of this division (relating to Display of License).
