SECTION/RULE §558.509 - Survey of a Branch Office, Alternate Delivery Site, and Services Provided

(a) If an agency is applying for or renewing a branch office or alternate delivery site license, a survey covers all locations.(b) If an agency is applying for a license to provide more than one category of service, a survey covers all provided services of the agency.
