SECTION/RULE §559.39 - Voluntary Closure

(a) A provider must notify HHSC in writing at least five days before permanent closure of the operation.(b) The provider must include in the written notice:(1) the date of permanent closure;(2) the reason for closing;(3) the location of individual records, both active and inactive; and(4) the name and address of the individual records custodian.
