A facility must:(1) report to HHSC via the online portal, or by speaking with an HHSC agent at 1-800-458-9858, upon learning of alleged abuse or neglect of a client and submit an investigation report to HHSC no later than the fifth working day after the initial report;(2) maintain incident reports as required by §559.75 of this subchapter (relating to Client Records);(3) ensure the confidentiality of individual client records and other information related to clients; and(4) inform the client, or client's responsible party, orally and in writing of the client's rights, responsibilities, and grievance procedures in a language the client, or client's responsible party, understands.
