SECTION/RULE §564.404 - Licensure Renewal

(a) A license issued by the Commission expires two years from the date of issuance.(b) The licensee shall file a request for renewal and pay the renewal fee at least 60 days before the license expires. Failure to file the required renewal and pay the renewal fee as specified may delay approval.(c) The facility shall not provide services after the license expiration date unless it has submitted the application update and fee by the date of expiration.
