SUBCHAPTER D - FACILITY LICENSURE INFORMATION
SECTION/RULE §564.405 - Changes in Status

(a) A facility shall submit the appropriate application and fees and receive written approval before:(1) adding a new detoxification service;(2) adding a new residential site;(3) moving to a new residential site; or(4) increasing the number of beds in a residential program.(b) If the facility fails to provide the information the Commission requires to process the change in status application within six months from the date of application, the application may be denied. The facility shall not reapply for six months from the date of denial.(c) A facility shall also notify the Commission's licensure department in writing before adding a new residential service, day treatment service or outpatient service; adding a new outpatient site or moving an outpatient site to a new location; or providing services to a new age group or gender.(d) A facility shall notify the Commission's facility licensure department prior to, or immediately after, a change in the organization's name, closure of a residential or outpatient location, decrease in the number of residential beds or discontinuance of a service.
