(a) To request an appeal, the qualified party must:(1) complete the required form; and(2) send the completed form either via email to the email address or via mail to the mailing address designated on the form.(b) The victim, alleged victim, legal guardian, parent (if the victim or alleged victim is a child), and reporter may request an appeal by calling the designated phone number.
