The following fees are refundable under the conditions noted:(1) We refund the application fee if the issuance of the permit was late without good cause and the applicant requests the refund;(2) We refund the initial license fee if the application is withdrawn, or the license is not issued;(3) We refund the non-expiring license fee if the initial license expires without the non-expiring license being issued; and(4) We refund the amendment fee if the amendment is not issued.
