(a) You must provide information about each person that is a controlling person at your operation as defined in §745.901(a) of this title (relating to Who is a controlling person at a child-care operation?) when you apply for your permit.(b) After you receive a permit from us, you must provide us information about someone who is a controlling person at your operation within two days after a person becomes a controlling person.(c) To provide the information to us, you must either:(1) Enter the information on-line through the DFPS website; or(2) Submit a completed Controlling Person Form to your local Licensing office.
