To demonstrate your criteria for assessing satisfactory completion of the program, you must demonstrate the following:(1) A statement of the passing level of performance and copy of the assessment tool used for this process; and(2) A copy of the credential including name, address, and telephone number of the organization, effective date of the credential, and a statement of how long the credential is valid.
