SECTION/RULE §748.1347 - What must I document regarding a professional level service provider's participation in the development of an initial service plan?

(a) You must document the professional level service provider's:(1) Name; and(2) Date of participation.(b) The professional level service provider must sign and date the document. If the provider disagrees with any portion of the plan, the provider must document the issue(s) of contention before signing it.
