At the time of a discharge or transfer, you must document the following:(1) The date and circumstances of the discharge or transfer;(2) Date and time the child was informed of his discharge or transfer, if applicable;(3) For discharge, the name, address, telephone number, and relationship of the person to whom you discharge the child, unless the child legally consents to a discharge. If the child legally consents to a discharge and does not want to involve the child's parent(s), you must document this in the child's record;(4) The child's service plans while in your care for the past 12 months;(5) A list of medications the child is taking, the dosage, frequency, and reason the medication was prescribed;(6) Any treatment for a physical condition that is in progress and requires continuing or follow-up medical care; and(7) For emergency discharge or transfer, the explanation given to the child regarding the reason for the discharge or transfer and the child's reaction to the discharge or transfer.
