SECTION/RULE §965.3 - Information Regarding Electronic Monitoring

(a) An individual or an individual's legally authorized representative must complete and sign the required Texas Health and Human Services Commission form pertaining to placing an electronic monitoring device in an individual's bedroom upon admission to a state supported living center.(b) A facility must maintain a copy of the completed and signed form in the individual's active record.
