SECTION/RULE §11.701 - Schedule of Charges Must be Filed Before Use

(a) No schedule of charges, formula, or method for calculating the schedule of charges may be used until a copy of the formula or method for calculating the schedule of charges with supporting documentation has been filed with the commissioner, as required by §11.703 of this title (relating to Filings and Supporting Documentation).(b) The schedule of charges must include all charges made for group, conversion, or individual coverage.
