(a) Applicants must submit the IRO application form for an original application for, renewal of, and reporting a material change in an IRO application form for a certificate of registration as an IRO in this state in the format prescribed by the department.(b) The commissioner adopts the biographical affidavit by reference to be used as an attachment to the IRO application form.(c) The forms are available at www.tdi.texas.gov/forms. Applicants may also obtain the forms from the Texas Department of Insurance, Mail Code 103-6A, P.O. Box 149104, Austin, Texas 78714-9104.
