SECTION/RULE §12.403 - Fee Amounts

(a) Fees to be paid to IROs by utilization review agents and other payors for each independent review are as follows:(1) tier one: $650; and(2) tier two: $460.(b) The IRO fees specified in subsection (a) of this section include an amended notification of decision if the department determines the initial notification of decision is incomplete. The amended notification of decision must be filed with the department no later than five working days from the IRO's receipt of notice from the department that the initial notification of decision is incomplete.
