The Department shall collect, and the person affected shall pay to the Department, the following fees:(1) $400--Initial license investigation fee;(2) $200--Initial license fee for licenses issued January 1 through June 30;(3) $100--Initial license fee for licenses issued July 1 through December 31;(4) $200--Investigation fee for change of ownership;(5) $200--Additional location license fee;(6) $20--Relocation fee;(7) $20--Name change fee;(8) $20--Duplicate license fee;(9) $200--License Renewal fee.
