(a) A Provider Network Contracting Entity Registration Form under §3.9802 of this title or a Provider Network Contracting Entity Exemption of Affiliates Form under §3.9803 of this title must be accompanied by the required fee of $1,000.(b) No fee is required for submitting a change in information under §3.9802 of this title or the annual registration update under §3.9803 of this title.
