(a) A person seeking a certificate of authority to act as a captive insurance company must submit an application and payment of the application fee in a format acceptable to the department and provide the information required in §6.202 of this title (relating to Captive Insurance Company Certificate of Authority Application Contents and Process).(b) The application fee is $1,500.(c) The department may provide forms that an applicant can use for submitting its application.
