SECTION/RULE §251.218 - Correspondence
Chapter Review Date 05/05/2000

All correspondence with the State Office of Risk Management concerning a claim must contain the following information:(1) the name of the injured employee;(2) the social security number of the injured employee;(3) the date of injury;(4) the name of the employing agency;(5) the director's claim number and the commission number, if known.
